Gynecological Patients

• We are currently accepting new gynecological patients at all of our many Locations.
• You can be seen by a physician, a midwife, or a nurse practitioner, depending on your preference.
• We provide a full range of gynecological Services, including Advanced Laparoscopic Surgery, Novasure Endometrial Ablation, Infertility Diagnosis and Treatment, and Menopausal Management.
• We can provide an appointment within 2 weeks for a new patient for routine exam or within 24 hours for those with an urgent issue.
• We accept a variety of Insurance plans.
• Prior to your appointment, please print out and fill out our Health History Form  and Patient Registration Form . We ask that you bring these completed forms with you to your first appointment.
• If your insurance requires a referral, you must give one to the receptionist on the day of your appointment or you will be responsible for the fee.
• Please be advised that there is a $50.00 charge if you miss your appointment or cancel with less than a 24 hour notice.
